HealthPathways Murrumbidgee facilitates communication and collaboration between health professionals across primary and acute care settings and encourages greater understanding between the sectors to improve the journeys of all patients through our local health system. The HealthPathways team is keen to hear from general practitioners, hospital specialists, nurses, allied and other health professionals who are interested in being involved.

HealthPathways Murrumbidgee Team

About us...all team members are based across the partnership (Murrumbidgee LHD and Murrumbidgee PHN) and work together as the HealthPathways Team.

  • Clinical Leader Work as part of the HealthPathways Murrumbidgee Team to develop and implement the HealthPathways Program. The role draws on clinical and healthcare system knowledge and experience to guide the localisation of the HealthPathways, and support discussion across professional networks to inform pathway development and implementation
  • GP Clinical Editors - Responsible for localisation of HealthPathways content and participating in clinical working groups and the effective implementation of approved pathways into existing practice. 
  • GP Localising Clinicians – responsible for ensuring that regional referral information is included in each pathway developed.
  • HealthPathways Coordinator / Manager- Responsible for supporting development, implementation and maintenance of the HealthPathways program.
